1. All eligible and affected customers who hold valid non-Wonder transaction records and verified receipts issued by AMG, and whose transactions cannot be verified by Wonder and/or who are unable to provide valid Wonder payment proofs.

Starting today, individuals can purchase and receive vaccinations not completed at AMG at a 50% discount ("this offer") at Medtimes.

2. This arrangement applies only to transactions for which Wonder receipt verification is unavailable, including those conducted via third-party non-

Wonder Payments, a payment service provider, has successfully completed a client's payment.

Eligible applicants must submit an application form via https://amgincidentbookingform.com and provide appropriate supporting documentation (e.g., AMG booking records or receipts). For the avoidance of doubt, eligible affected customers must submit the application form and relevant supporting documents for verification by Wonder on or before September 7, 2025, to qualify for this offer. Customers who have previously submitted an application form and are eligible will be automatically enrolled in this support program offer.

4. This offer does not apply to eligible customers who have already completed Wonder transaction verification or hold a valid Wonder payment voucher. These customers may still submit appropriate supporting documents via https://amgincidentbookingform.com for verification by Wonder. Upon successful verification, they will be able to receive complimentary vaccination services at other partner medical institutions, subject to the availability confirmed by Wonder for those specific medical institutions.

This offer is valid only for registrations made on or before September 7, 2025, and is applicable exclusively at Medtimes Healthcare Service Centers.
